All students on Haverford affiliated, funded, or sponsored travel are required to complete and return an Assumption of Risk, Waiver of Liability, Release, and Indemnification Agreement form.
If a student is studying or working abroad in a country that is on the US State Department Travel Warning list or if a student is under 18 years of age, this form must be submitted in hard copy to the office or department with which they are traveling, with a parent or guardian's signature.
If you decide to extend your stay and choose to do some personal traveling after the program/internship has ended, you are not covered by the College’s insurance, and the College is not liable for personal safety, health care, emergency evacuation or other services.
Deadline for the submission of this form is Dec. 1 for spring semester travel and May 1 for fall, year, or summer travel.
