Some of the forms and links below require your Haverford Student ID.
Insurance: Enroll or Waive (Required)
All students are required to have personal medical insurance and must provide proof of insurance via the HaverHealth Portal. The College suggests purchasing the Student Health Insurance Plan (SHIP) if you have a high deductible, HMO, or out of state insurance plan that does not work well in the Philadelphia area. If purchasing the SHIP, print out your card and upload to the HaverHealth Portal.
Each year, all new incoming students, returning students, others returning from leave, and new transfer students will be automatically billed the SHIP premium charge by the Controller's Office. Students must access the First Student site and enroll in order to print an insurance card for the Haverhealth Portal. Students must complete a waiver at the First Student site to have this charge removed. International students are automatically enrolled in SHIP and may not waive their coverage.
If you have any questions regarding SHIP billing, including the processing of a waiver, please review the information provided on the SHIP details page of the Health Services site. Those students who do not complete a waiver by the August 2, 2019 Tuition payment due date will be expected to pay the insurance premium.
Enroll or Waive Insurance Deadline: July 22, 2019
Note: The Student Health Insurance Plan (SHIP) is administered by First Risk Advisors and is underwritten by the United Healthcare Insurance Company with an annual policy period of 8/1 to 7/31. The premium for 2019-2020 is $1,782. For more information on the SHIP and its benefits please visit firststudent.com. If you have questions, please contact First Risk Advisor's Customer Service at 800-505-4160 or firstname.lastname@example.org.
Health Records (Required)
You'll be able to access and submit the matriculation forms required by the College using the HaverHealth Portal. You will be able to access the HaverHealth Portal within 48 hours of the time you activate your Haverford account in early June.
If you have any questions, email email@example.com, call (610) 896-1089, or send a fax to (610) 896-1090. The clinic is closed during summer but emails/voice mail/mail/faxes are processed weekly.
Health Records Deadline: July 22, 2019
A late fee of $75 will be charged for failure to submit your fully completed health forms by the deadline.
Note: The ID used for your Health Records is different than that used for your Insurance.
Consent Tutorial (Required)
Sexual assault and sexual misconduct are acts of sexual violence that violate Haverford's foundational principles as embodied in our Honor Code. They have no place at Haverford. That said, we do not live in a bubble, and so are not immune from such behaviors. We must invest time, energy, and vigilance in our continued efforts to keep our community safe from sexual misconduct and sexual violence. We want students to have the knowledge and skills to understand the meaning of consent as well as to be an active bystander, who intervenes when anyone is observed being harassed or endangered by sexual violence.
How to Complete the Training: You will receive an email to you Haverford Email Account from our training partner, Get Inclusive, in mid-June of 2019. Simply follow the link in that email to begin the training. The training module is self-paced and will take around 20-30 minutes to complete.
Consent Tutorial Deadline: August 15, 2019
Note: You will receive an email in mid-June.

Record Your Name Pronunciation (Optional)
On July 1, you'll receive an email with information about recording your name pronunciation from a system called Name Coach. It’s not phishing, or a scam (but it’s good to be vigilant!); it’s just an external platform Haverford uses. Name Coach is a project from the Council on Diversity and Inclusion and the Provost's Office, allowing you to record the pronunciation of your name for others to hear, and then, to pronounce correctly.
Name Coach is integrated into Moodle, the course platform we use, and you will see links on the class pages to access all recordings from students in that class. If you explore the Name Coach site, and the "Name Badge" page in particular, you’ll see a number of features, including the ability to insert the recording of your name into your email signature. It’s also helpful if you create a password for convenient access to your NameCoach account in the future.

Record Your Name Deadline: August 1, 2019
Religious Affiliation Survey (Optional)
Religious and Spiritual Life on campus at Haverford College is supported through the Quaker Affairs Office. As they work to support students of faith, it is helpful to know the numbers and the breadth of religious practice on campus. Response to this survey is strictly optional and your specific information will only be shared with your permission.
If you have any questions about the survey, about Quakerism, or religious life on campus, connect with Walter at firstname.lastname@example.org.
Religious Affiliation Survey Deadline: July 22, 2019
