Some of the forms and links below require your Haverford Student ID.
Insurance: Enroll or Waive (Required)
All students are required to have personal medical insurance and must provide proof of insurance via the HaverHealth Portal. The College suggests purchasing the Student Health Insurance Plan (SHIP) if you have a high deductible, HMO, or out of state insurance plan that does not work well in the Philadelphia area. If purchasing the SHIP, print out your card and upload to the HaverHealth Portal.
Each year, all new incoming students, returning students, others returning from leave, and new transfer students will be automatically enrolled in SHIP and billed the premium charge by the business office. If you have any questions regarding SHIP billing please contact, Michael Gavanus (firstname.lastname@example.org).
Deadline: July 21, 2018
Health Records (Required)
You'll be able to access and submit the matriculation forms required by the College using the HaverHealth Portal. You will be able to access the HaverHealth Portal within 48 hours of the time you activate your Haverford account in early June.
If you have any questions, email Catherine Sharbaugh (email@example.com), Director of Health Services, call (610) 896-1089, or send a fax to (610) 896-1090. The clinic is closed during summer but emails/voice mail/mail/faxes are processed weekly.
Deadline: July 21, 2018
A late fee of $75 will be charged for failure to submit your fully completed health forms by the deadline.
Note: The ID used for your Health Records is different than that used for your Insurance.
Sexual Violence Tutorial (Required)
Sexual assault and sexual misconduct are acts of violence and disrespect that violate Haverford’s foundational principles as embodied in our Honor Code. They have no place here. That said, we do not live in a bubble, and are not immune from such behaviors. Reports of sexual misconduct are infrequent, but that does not mean it cannot happen at Haverford. National statistics show that first-year students are particularly vulnerable during the first months of college. It is not surprising, then, that we will be spending a good deal of time during Customs Week on this very important issue so as to achieve our goal of preventing sexual misconduct to the greatest extent possible.
Some of you may already be very familiar with and knowledgeable about many aspects of this issue. Others may not be. To ensure that all incoming students, who include first-years, transfers, and exchange students, can come into these discussions with a common base of knowledge, we are asking that you complete a required, 45-minute online course entitled, “Impressions,” which is designed to help you recognize sexual assault—what it is, how to help prevent it, and how to report it—and inform you about the support resources available, both on and off campus.
Deadline: August 20, 2018
Note: Account creation email will be sent in early July, 2018.
Tips for Completing Courses
- In early July, you will receive an account creation email. This will contain your username and a temporary password. The first time you log in, the system will prompt you to reset your temporary password and set a security question.
- Pop-up blockers must be disabled.
- Ensure your computer meets all System Requirements: Test Your Configuration
Record Your Name Pronunciation (Optional)
On July 1, you'll receive an email with information about recording your name pronunciation from a system called Name Coach. It’s not phishing, or a scam (but it’s good to be vigilant!); it’s just an external platform Haverford uses. Name Coach is a project from the Council on Diversity and Inclusion and the Provost's Office, allowing you to record the pronunciation of your name for others to hear, and then, to pronounce correctly.
Name Coach is integrated into Moodle, the course platform we use, and you will see links on the class pages to access all recordings from students in that class. If you explore the Name Coach site, and the "Name Badge" page in particular, you’ll see a number of features, including the ability to insert the recording of your name into your email signature. It’s also helpful if you create a password for convenient access to your NameCoach account in the future.

Religious Affiliation Survey (Optional)
Religious and Spiritual Life on campus at Haverford College is supported through the Quaker Affairs Office. As they work to support students of faith, it is helpful to know the numbers and the breadth of religious practice on campus. Response to this survey is strictly optional and your specific information will only be shared with your permission.
If you have any questions about the survey, about Quakerism, or religious life on campus, connect with Walter at email@example.com.
Deadline: July 21, 2018
