Print out this page. We MUST have a signature or the form cannot be processed.

• Fill out the requested information.  Please use complete addresses. If the confirmation should go to the same address as the transcript, you may indicate that by entering your address in the Send Transcript To: block and entering "SAME" in the Send Confirmation To: block.
• Please use one request for each Send Transcript To: address.
• Transcripts fees are $5.00 per copy, payable with request.
• If you wish to have the envelope signed across the seal, please note that under Special Instructions.
• If you studied abroad while a Haverford student and wish to have a photocopy of your study abroad transcript included with your Official Haverford transcript, please note that under Special Instructions. Photocopies will only be provided for study abroad transcripts issued outside the United States. i.e. If you studied abroad in Japan through Temple University's study abroad program, the transcript is issued by Temple University and therefore a photocopy cannot be provided.
