The white patch and the scabs in the nostrils remind Freud of the diphtheria of both Irma's friend and his own daughter Mathilde, and of his fear that he may have damaged his nose with cocaine use. The theme of cocaine calls up Freud's recommendation of cocaine in 1885, and reminds him of Flieschl's death, for which he reproaches himself.
