How to obtain copies of medical records.
Current students and recent grads can get copies of their medical records from Health Services.
If you are a current Haverford College Student you can stop by Health Services and sign a form for release of information.
If you have graduated in the past seven years your records are still available. You must either mail or fax a request. You must state your name and year of graduation. You must state exactly what records you are looking for. You must state where you want the records mailed or faxed to and a number where you can be reached.
There is a five dollar fee. The check must be made out to Haverford College and mailed to:
