Because it may take several weeks to be seen by your healthcare provider, we strongly recommend that you schedule a physical exam as soon as possible.

Incoming Students

You will need your student ID number to purchase/ waive insurance or complete Confidential Health online form.

You will need your student ID number to complete forms:

• Insurance Enroll/Waiver (Due August 1)
• Confidential Health Record (Due July 21)
• Sickle Cell Education with waiver or testing (Due July 21) - Contact Greg Rosnick at 610-795-6129 with any questions
• Students under 18 will need to submit the Consent to Treat a Minor form
