Your Health Record is a private matter between you and your health care provider. Information can only be released with your written permission. This means that we cannot give any information to your friends, parents, customs people, advisors, professors or hometown physician without your express written permission and a signed Authorization of Disclosure of Information Form as required by law.
- When a student is 18 years old, parents or guardians are only notified only in case of a medical emergency.
- Students over the age of 18 must sign a authorization for disclosure of healthcare information for Health Services to release medical information.
