Confidentiality

Your Health Records is a private matter between you and your health care provider.  Information can only be released with your permission.  This means that we cannot give any information to your friends, parents, custom people, advisors, professors or hometown physician without your expressed written permission and a signed Authorization of Disclosure of Information Form as required by law.

Parents or guardians are notified only in case of an emergency.

Please note: Students over the age of 18 must sign a authorization for disclosure of healthcare information for the healthcare provider to release medical information.
