Programs Funds Request Process

1. Complete this form by October 9 for Fall programs/November 25 for Winter programs.
2. Make an appointment to meet with Frederic to discuss the program once the application is complete.
3. A decision regarding granting of funds will be delivered within 72 hours after your appointment with Frederic.

Post-Program Requirements

1. Submit receipts to Deborarh 24-48 hours following the program.
2. Submit program evaluation form (coming soon) to Deborah 24-48 hours following the program.
3. Make follow-up appointment with Frederic to discuss what went well and what was learned fromt he program. This should happen the week following the program.
