Video Production Equipment Reservation Form
NOTE: This form and equipment are ONLY for the use of students registered in Anthropology 358 (ANTHH358A01, Anthropology of Capitalisms, Maris Gillette) during the Fall '09 semester. If you are not enrolled in this class but need to borrow video equipment please inquire.
Reservations- Reservations must be made in advance by filling out and submitting this form. We recommend at least 7 days notice. The sooner you get your request in the better. Last minute reservations can not be honored.
- You must make an appointment with us to both pick up and return the equipment. Plan accordingly. We are open from 9 and 4:45, Monday through Friday. We are often out of the office and, without an appointment, chances are we will not be here when you just "stop by". Our office is in Stokes 020, in the basement level.
When you make your reservations you must:
- Indicate, in general, what equipment you will be borrowing. (We can adjust the equipment you are borrowing when you come to pick it up.)
- Tell us the specific day and time you will be picking up the equipment.
- Tell us the day and time you will be returning the equipment. (If you are not sure, guess. You can adjust later)
