Medical histories and annual updates are needed for every student-athlete wishing to participate in intercollegiate athletics at Haverford College.

This information will be maintained in a strictly confidential manner. The information provided is solely for the use of the Athletic Training and Health Service personnel.

Please be as accurate as possible when answering the questions. In order to participate in the athletic program you must submit the requested information in a timely manner.
